I understand now that the lightning bolt means top 5 poster for the "past month" but I am unsure how often this is updated and when. Is it actually updated monthly and if so is it the first of the month ?

Crown means Administrator.
Shield means Moderator.
Lightning bolt means Top Poster
Diamond means Diamond.

Top Posters is updated real time, with the exception of caching delays. It doesn't go by calendar months, but by the past 30 days (i.e. since December 10th).
