.Open a new document of 400 x 500.And using the rectangle tool make a shape like this of any color.
.Now in the foreground color use this color #000000 and the background color #8a8a8a. Now right click on the shape -> Blending options -> Gradient overlay -> Choose the above two color's gradient and keep the rest like this.
.Layer -> Restersize ->layer
4. Edit -> Transform -> perspective . Hold on the lower left sqare and move it rightways to get the shape. Right click on the layer again -> blending options -> bevel and emboss and then use the settings.Also add drop shadow.
.Select -> load selection(of the shape layer) -> Make a new layer -> now go to select -> modify -> contract by 1 px . Get hold of a round brush and in the new layer make The shape as in the pic(The selection must still be there). Reduce the opacity of the layer to 24 %.
.Make a new layer and the using the men tool make a selection and fill it with white color.Reduce the opacity to about 14%.
.Now make another rectangle layer with foreground color #646464.The layer should be in between the first shape and layer 0.See the image.
.Make a new layer.Now again use the pen tool and make this shape at the bottom.And fill it with a color of your choice.
.Now write Your text(using image is a different matter) Fill opacity -0 and then go to blending options- > outer glow .
