I understand you are probably very upset with her atm but you have to realize that she probably doesn't even want to do it. Heroin has a nasty addiction that is next to impossible to shake.
The only thing you can do is love her to bits and pieces and try to stay with her through her time of need. She will love you for it and later on in life.
Try and stay with her all the time and take her places to keep her mind off of it, if you can consistently stay with her for a couple weeks and make sure she doesn't get any it will go away.
If she starts looking very ill and sweating or shaking or anything just give her a huge hug and cuddle with her until it stops!!!! shits rough man i know.

There has to be a certain point in your life where you need to choose yourself over these types of people because there are seldom any stories of people successfully dating junkies. At a younger age(sub 30's) it's easy to move on and do something better with your life. I've always been a firm believer in "You're Only As Good As Who You Surround Yourself With". Also I want to say I think dropping someone who is a junkie for self preservation and long term happiness isn't selfish.
You see I somewhat agree, I mean if it was a drug like alcohol or something of that nature it would be a completely different story for me.
But when you look at the physical addiction of heroin and how bad withdrawls are and how sick it makes you without it, its alot harder.
Especially if she has used for a long time now, I mean if you love her you cant just give up on her when she needs you most!
PS. depending on how long she was in rehab for,
I mean if she was there for a month then was released and used heroin then yeah it may be a lost cause
