I can't recall it happening to any large organizations here, but it definitely happens everywhere. There's also the possibility it was a disgruntled/unstable employee.

Well yes, we both have no doubts about it... however, when things like this happen under our nose, they always lead to "moments of reflection".
Personally, I believe that it should be the writer's skill to keep the engagement high, using such "aids" to boost it is pretty sad...
