How old are you? At least roughly, if you do not wish to share your exact age. It's not very common for really young people to have a heart attack and pain could indicate a lot of other things. Sometimes such pain can stem from problems with your back, especially if you're not sitting properly or standing up right for a prolonged period of time. It is also quite common psychosomatic problem with onset of some anxiety disorders, usually when you get first panic attacks, many people (including me, years ago) think that they're having an heart attack (or some other problem) during their first panic attacks (well even later ones sometimes...) combined with extreme fear from death and dying, often during night.

However, the best advice you could get here is: get yourself checked by a doctor. Nobody here can help you more than a doctor, even if somebody here was a doctor, as it requires some physical examination and such.

I don't think they need an ECG to determine if you're having an heart attack. If you actually have an heart attack, you'll most probably now by the sharp pain and other acute symptoms and an action needs to be taken immediately (I think they use some solution that dissolves blood clots), because when you do have heart attack, one of the veins that supply oxygen to heart is blocked and basically part of the heart tissue dies from the lack of the oxygen.

It is possible to determine afterwards if you had an heart attack by bloodwork, as it causes increased levels of some chemical (I don't remember what exactly it is) in the bloodstream for some time after the incident.

However, you may have some symptoms prior to having an heart attack, usually when the bloodflow is severely restricted, usually by cholesterol/fat (and part of the heart doesn't have enough oxygen), so it might cause such symptoms with increased physical load (the heart needs to pump faster - needs more oxygen - isn't getting enough). I'm not sure how they determine this though, but if you already ARE having a heart attack, it is usually too late to do any tests and you should be treated immediately. Unless it's a mild one, I read that some might not even have any symptoms at all, but I don't know much about that.

Plus there are other things that can cause such symptoms, various diseases and such. It could be also entirely psychical, as psychical problems can manifest themselves in variety of physical ways (I have my own experience with this) apart from the psychical ones.

However, I don't know much about this, I'm not a doctor, this some of what I learned myself when I was reading various sources (partly from interest, partly from paranoia), so again, you should check with a doctor.
