Looks like you're importing too many fields into your table.
Your table has 6 fields. You're trying to import 9 fields.

I could be wrong.

The note field is empty in the second set of values (second set of brackets) ... it is set as '' whereas in others it includes data such as 'Nely' or whatever. Here is what it should be, I changed the missing note to USERNAME for example purposes.
