Maybe for you, i found a few months ago a hacking forum "FraudsterCrew" and they did have Adsense on the forum. I report to Adsense support - nothing happen.
it all about luck sometime .it already happen i was not expecting to get approval on my hacking blog .after i decided to change adsense account and from there i got caught and never approved.

No it's a mix. Of Adwords spending and Adsense earning. If the money is legit they will allow it.

Everything is anecdotal but I'd say the higher profile the site, the more likely ads will be taken down. Their terms of service do prohibit certain sites, so we knew it was a risk when we used them. I'm glad they gave us the push, as we have benefited much more from running ads independently.
