I have zero clue what you're even talking about. If you mean that "Created By Ayman Eldoctor Copyright 2009" doesn't show up on the form when you downloaded that project, then you shouldn't even be trying to use someone else's source code because you don't know what you're doing in the slightest if you can't figure that out.

And if you cannot do that, then I doubt you can "build" your own programs. You need to start from the basics and move up from there. As far as I can tell, you haven't been getting anywhere though, and you won't if you try to use source code that you don't understand either.
