[*code] <- denotes the start of your code
Enter your code here <- keeps the correct formating and sets the code in a block
[*/code] <- denotes the end of your code

just remove the *'s

I don't code in PHP so I will be unable to help you with the code, however now that your code is correctly formatted and easy to read it will be easier for people to see what is wrong and point you in the right direction.
