Okay dude, your code is very confusing.

What are you defining your form element as?

Idk if its the site itself that is just not showing how it looks after I try to modify it or what.

I'll start into fixing it, at the very least, I've fixed a few errors/possible design flaws.

I might be mistaken but you may also be repeating elements without modifiers as well.
