Can I put a hack on a cd-r, so when I put it in someones computer, it will start installing a, rat for example ?

Sure you can. If Autorun is enabled on that someone's computer.

So I create autorun.inf , the files etc, and just burn them in to the disc?

It's the same for every drive. Even your HDD partition can be Autorun.inf'ed
